We are very excited to have you become a part of our parish. Simply fill out the information on the form, and you're all set! You can also use the form to update your current profile. We will never share your information, nor use it for personal use. If you choose to select envelopes, they will be mailed out to you on a quarterly basis. Please contact the office by phone or email if you have not received your envelopes.
